We make every effort to ensure that any information held is accurate and as up-to-date as possible.
It is not Janice Andersen policy to seek any sensitive information unless legally required in order to diagnose, recommend or deliver the particular
service or treatment or provide recommendations e.g clincial tests or products. Sensitive information includes data relating to physical or mental health,
